Doctors Visits, How do you do it???

How do you do doctors appt./dental appt.? My son is PDD-nos and mood disorder-nos. Since he was little going to the doctor/dentist has been hell. He runs crazy, throws things, he doesn't want the doctor to touch him. He hits me, tries to dump out my purse, untise or take my shoes off. Forget getting shots, it takes up to 4 nurses to hold him down. Even a simple finger stick and it takes that many people to hold him down. He is seven and hasn't had his teeth cleaned he refuses to let them clean them, if they get him to open they can look for only a minute. They bribe him with everything they have, I try to bribe him. He leaves with all kinds of stuff, but never gets his teeth cleaned. They try to so hard and are so understanding. I don't know what to do. Does anyone have advice, how do you do it?

Here is a lil trick I use when I have to stick a kid. I get a sheet, fold it in half length ways, we play a little game where he is the burrito and we wrap him up super tight with one arm out. One person the kid down, one person hold the arm, one person hold the needle. He can't move, all he can do is scream, and that wont hurt anything so just let him scream until they are done. Good luck with the dentist, we have to sedate my son for his cleanings.
