I don't know if you've been able to address this problem yet with your son, so I thought I'd give my 2 cents.

My daughter used to hold it in too long as well, as hers tended to be large and hurt her. Her pediatrician suggested we start with 1 tsp of Karo syrup in her juice each morning (2 tsp max. per day if needed!) After a couple days of drinking it, she couldn't help but have to go. The Karo syrup is tapered off slowly, and we rarely need to use it now. Also, when she did sit down (in the beginning) I softly spoke with her about that it may hurt coming out at first but if she continued to go regularly it wouldn't hurt any longer. We made a big deal about her finally pooping EVERY TIME. It's a tough road at first, but believe me it's worth getting your child to learn to let it out when they need to. I hope this helps.
