Depends on your situation. I wish I didn't put my daughter's bio father's name on her birth certificate. Not because I want to hide it from her. She's 5 and knows as much as she can understand about her "father" but because everything would be so much easier if he wasnt listed. He wants nothing to do with her but I have to have his consent for everything because he is listed and he wont give me consent. My husband, whos been daddy since she was 2 wants to adopt her but cant because of her "father". Sad situation.

In most states, the father must be present and authorize that he be designated as the father on the birth certificate. Otherwise, it may be left blank and the paternity established via the courts.

I say put it on because it is okay to have the name on it and if you were to seek child support it may just make the process easier. What you should not do is give the baby the dads last name. That is sure to cause an issue later on. I regret giving my son his fathers last name, oh what a headache it is with certain things to get done, esp with the passport.

You don't have to do anything. No one can make you fill it out.

In California, the father has to be present in the hospital to sign the birth certificate, You can not put their name there on your own.

Nope you can leave it blank. I reccommend putting it on though because av few years ago my ex husband had to get a replacement social security card and the office asked for his fathers name and he had no idea who it was and it was a huge hassle to get the card because of it. Apparanetly you need to know what the birth certificate says.

No you can leave it blank if you want to but it does hold problems later if you do this. I am not sure where you are in the world but some countries only allow child maintenance to be paid from the birth certificate. If the father is not listed you can not legally request payments.
Also it is not very nice for the child to have father unknown on their birth certificate. If you know who the father is then put there name down. It is only fair for that child to know the name of their father even if they are not going to be part of thier lives. They have a name to find them later if they want to as an adult. We all need to know our roots!
