My 10 year old gifted son with ADHD is falling very far behind in 5th grade. He is more than capable of the work. He just isn't able to stay focused long enough to compete his school assignments. He has not been bringing home all of his homework. The teacher wants him to be responsible to write down and turn in his homework. If he forgets his name or date it gets returned and then is late and he only gets 50 % credit. He has had several assignments he did at home but forgot to turn in. That also gets 50% credit.He is extremely hyper and takes 20mg of Adderal XR in the AM and 5mg of intunive 2x a day. We are scheduled to see his doctor next month to see about medication change or increase. We have never had to ask for a 504 before and I am not sure what I should request. I want to ask for extra books at home and for the teacher to keep me informed of daily homework/class work that needs to be done, Any other suggestions. He looses recess 3-4 times a week but I don't know if they will let me around that one. Although I know he will be better behaved in the classroom with that physical outlet. Luckily we are in a very good school district and so far they have been helpful.(I've read some real horror stories in the blogs)
