Suggestions on how to stop a 5 year old from wetting himself?

I am extremely new to this,but need help bad. I have 4 children. Two biological and 2 step. We have had custody of my step children for 4 years now and they have extremely little contact with their biological mom, (she's in jail). About 2 weeks ago my 5 year old step son began to urinate on himself, in his bed, in his brothers bedroom, just everywhere. He is wide awake and knows completely what he is doing. He has been pooty trained for 3 1/2 years now. I have never had to deal with a situation like this in my two older children and do not know what to do. Does anyone have any suggestions?

He is an extremely lovable young man, polite, and helpful, the only concern I do have with his behavior is he is 100% of the time "glued" to my leg. Unlike his brother and the experiences with my older kids who are able to entertain themselves whether it be, video games, television, conversation with each other (normal childlike behavior) He doesnt do that. If I have to use the restroom, he sits at thte bathroom door and waits until I am done, same as when I take a bath. During school outings (field trips, field day etc...) when I show up to support him and his class, he stops playing all together only to be by my side.

Take him to the doctor to rule out an infection. If that isn't the case, then I'm not sure. Could be emotional due to his bio mom's circumstances. Could be something else entirely. How is the rest of his behavior?
