My 5 yr old son is very fidgity - but not all the time. He sometimes gets excited and hugs/tackles his friends or gets in their face and his pre-school teacher seems to think there is something wrong with him. He has no problem playing alone nor concentrating for long periods. He can sit quietly while I read him a book, watching a movie or playing a video game. He even likes to practice writting his letters.

I'm concerned because he starts kindergarten in the fall.....

My son is the same way. I took him to a occupational therapist who diagnosed him with a sensory processing disorder. He started kindergarten this year and with a patient teacher he's doing great. I hope this helps.

It could be that he gets overstimulated in crowds, but when he is alone with you, there is no one else to grab his attention away from what you are doing. I would get him tested for Attention Defecit Disorder and even Attention Defecit Hyperactivity Disorder. I went through the same thing with my son, who is now a well adjusted 24 year old man. He acted very much the same way as your son does. One-on-one he was fine, but add another one or two people to the mix and watch him go into hyper gear! Good luck and I hope you get this figured out.
