We had the same problem about 6-8 weeks ago. I had to completely eliminate dairy from my daughter's diet AND switch to soy. It took about a week, maybe week and a half, but it did finally work. We had to give her plain, plain, plain food. Chicken sticks, applesauce, bread almost every meal and of course the soy milk/soy formula. Our doctor has studied under several pediatric specialties and explained that one small stomach bug can cause a lactose intolerance for up to a year and a half. Again, after that week or so it finally worked. Now, any time she accidentally has dairy she gets diarreah, but as long as she has no dairy, she's good to go. Hope this helps!
