Twin boys, one with disruptive behavior problems in kindergarten. Need advice!

I have identical twin boys in kindergarten. One of them is having severe behavior issues. So severe that he hit his teacher today and we had a meeting that if he does it again, he will be kicked out of school. The problem is, if he receives any kind of discipline, such as moving his clip from a good behavior color to a bad behavior color, he flips out. He does not think of consequences no matter how many discussions we have about it at home. He gets mad and just loses it. For disciplinary action, he loses privileges, has his bedtime moved up, and has been made to write better choices he could have made during other instances. He seems to understand and a couple weeks later, does it again. His twin on the other hand, is respectful, gets rewarded constantly for good behavior, and is thriving in his class. I need advice!

He needs to be seen by a doctor. Because his behavior is so extreme it sounds to me like there may be something going on with him internally. Especially since you have twins and the other is fine. Has he been to see the school counselor? that might be a good place to start.

I actually did speak with his school counsellor the next day after I posted this. She was very helpful and we had a meeting with his teacher. We came to the agreement that I give too many 'chances' before disciplinary action and she asked his teacher to give one warning before disciplinary action, kind of a meeting in the middle. I asked her about seeing a doctor and she told me she does not think he is malicious in his behavior, but does not like being called out when he is not doing what he supposed to. She suggested not to make an appointment yet and gave tips on how situations should be handled, but to mention it at his next check up if I think necessary, or make an appointment if he does not improve. Thank you for commenting, I'm open to all suggestions.
