My son will bang his head so hard against walls that he gets bruises. It's pretty rare now; was worse when he was teething a lot and going through some major growth/development stages. I think they just don't understand what they're doing! They don't realized that biting themselves will make them bleed and there will still be a mark later. It's just a way of releasing some major energy/emotions. They can also feed off their own tantrums so it escalates: being upset makes them more upset because they can't control it yet! When my son gets so caught up in his tantrum that I think he'll hurt himself I do something to snap him out of it. Clapping loudly, suddenly opening a window, that kind of thing. If that doesn't work I hold him so he can't hurt himself until he's calmer.
Try to notice the circumstances around your kiddo's worse tantrums. Did he eat/wear/do the same thing before each? Maybe it's due to a food intolerance that hurts her inside, missing a nap, having the normal day's routine disrupted, being around lots of noisy people, etc. If you're really worried I would call your pediatrician or even the local WIC office. They can refer you to someone who knows more about it. Good luck and stay cool :-)

Wow that must have been so scary to watch! I haven't experience anything like that but thought I would give you links to some articles I read recently. They explore tantrums and aggressive behaviour in children - I think the same would apply only you would need to provide a loving limit to stop your child hurting himself rather than someone else. http://www.handinhandparenting.org/news/... &

Wish you all the best
