What could cause a toddler to wake at the exact same time every night?

Last week my son started waking up at 4am sitting up and moaning and then standing in his crib and crying. On the same day his naps went from 90 min to 40 and he wakes up the same way. Physically the Dr says he is fine, no ear infection, etc. He has all molars and only one cuspid left to come in. He's 19 months.what can be causing this? It feels like we have a 3 month old again.

My son did this....It is just a habit, maybe even growing pains, or nightmares.....For us we began to now which of those it was and most of the time it was just habit. We finally and reluctantly stopped going to him (unless it was a terror cry) and he would fall right back asleep within a minute or so. When we do attend to him we rub his back just for a few minutes and quietly leave. Worked great for us!
