My 4.5 year old son (B/G twins) has just been diagnosed with childhood apraxia as severe profound. He has a vocabulary of about 100 words and I am wondering if anyone has dealt with this and could give me some pointers of things we can do at home as he is going to a speech pathologist.

If your Speech therapist is worth anything, they should be able to give you tips, tricks, and "homework" for things to do to help at home. We focused on songs and books that one of my daughters loved, and would pick one word per page to really push her to say. There are exercised for each sound that you can do to "force" the sound. If your therapist isn't giving you home info, ask him/her for some!
