My 10 year old daughter has been diagnosed with ADD about a month ago. She has never had issues with behavior problems its her focus. She loses focus easily. Reading is the most challenging for her. She started Concerta 18 mg, the first three days we noticed a little more focus with regards to her completing her homework. Now, she has lost focus again, should her dose be increased to 27mg. Does weight have anything to do with dosage. She is 5'3 126lbs?

These are all questions you should be asking her pediatrician/doctor. You shouldn't be increasing dosage without having that conversation. But yes, things like weight, and even hormones, have to do with the right amount of medication.
