Yes. It is rare, but possible. The odds actually go up the longer it's been since you had it done because your body can sometimes heal it.
It is also recommended to use another form of birth control for the first 6 months to a year after you have it done.

Yup, definitely possible. My friend was a total oops. Her mom got her tubes tied after having her brother. Then 5ish years later, got pregnant with my friend.
I think that it depends a lot on how you got your tubes tied. I believe there's 3 different ways to get it done. Clipped, cut, and burned.
With clipped and cut there is a chance that the clip can come undone or that the cut tube can heal itself. I believe that burned is the most permanent method and the least likely to have an accident with.
