Extreme ADHD issues with my 6 year old

I have a son that apparently has extreme ADHD, regular ADHD apparently wasn't good enough. He is ABSOLUTELY DRIVING ME INSANE!!!!!!!!!!!!!!!!!!!!!!!! I am a working mom 40+ hrs a week and I take two college courses during the week as well and I have a 14 month old. He causes no harm to his brother and loves him very much. I started having him looked at when he was 3. They told me I had to wait til he was at least 5 to medicate him. Medicating him was my last resort. He doesn't throw tantrums like regular kids who roll on the floor, but he isn't one of those kids that if you ignore them they go away. He is persistant|!!! I eventually give in which I Know I shouldn't. but he aggravates the hell out of me to say the least. My fiancé works the graveyard shift and every weekend, so a lot of the time I have the kids by myself or at my moms house or his moms house when I have school. He CONSTANTLY TALKS. He never shuts up. I tell him several times a day to just stop talking and go away that he is driving me crazy!!! and it doesn't work! I am at my wits absolute freaking end! He doesn't have too many anger outbursts, but when he does its to himself and he just throws whatever toy has pissed him off. But none of the remotes in my house have backs because he has lost them from taking all the batteries out and putting them in his toys. So all of those toys have since disappeared. I've even gone as far as taking out every single toy in his room and thrown them in trash bags and put them in the basement, and it seems to have no effect what so ever. Not to mention now that he is medicated he keeps peeking on it and it keeps having to be upped. He is medicated three times a day and it still seems like it isn't enough. he always interrupts and is a constant aggravation and I feel like its wrong of me to feel this way this time of his life is suppose to be fun and play time and its nothing but stress and aggravation. all I do is yell. and I'm losing myself in the process. I'm one interruption or smart back talk away from sending his lil butt somewhere else to live because I'm on the verge of losing my cool and just using extreme measures or shipping him off somewhere. please help with pointers or anything anything at all

Wow did I write this? Scary though, my son is going on medication and I thought it would be the light at the end of the tunnel.
