I'm not certain that there is anything that can be done to prevent it happening. As far as I understand it, there is a risk of 1 in a 100 pregnancies having some kind of knot in the umbilical cord (known as a nuchal loop) and 1 in 2000 developing the tight knot referred to as 'true', which has such terrible consequences.
It was devasting for you to lose a baby in this manner, but hopefully you will never be so unlucky again as it really is just a matter of happenstance as to whether the baby wraps itself in the cord and then passes through the loop. You can cut down on some of the known risk factors by quitting smoking if you smoke, making sure that you have excellent nutrition etc. I think babies most at risk are large babies or multiples, but it would be best to ask your OB-GYN for more advice.
Certainly you should keep a kick count in the later stages of your pregnancy, because decreased kick activity is one sign that a nuchal loop may have formed. Really though, the development of this condition is so rare that I would hope most sincerely that you shouldn't have to go through this twice.
