I was a toe walker for years, and so was my oldest daughter. I actually ended up taking my daughter to the Shriner's Hospital because her feet got so bad. She complained that her heels hurt her if she was flat footed. Come to find out, because of her walking on her toes all the time, the tendon in the back of her foot shortened, and would cause her pain when she was flat on her feet. The therapy involved stretches every night. If that didn't work, she would have to wear splints for a while. Last resort was surgery. Luckily, all we ever had to do was stretches, but it was painful for a while. Good luck.

Can he stand/walk flat footed? If so... it's most likely just a stage. Pretty much every child I've come in contact with at that age has done some tiptoe walking. Apparently it's fun. ;)
If he can't stand/walk flat footed at all... there could be an issue that needs checked out.
