how do I get my 20 month old son to talk

My son is 20 months old and only says 1 word mama, and he doesn't say it all the time. He loves books and showing me things, he just won't talk. I'm afraid of autism spectrum disorder. He does this shaking thing and open and closing fists when excited. I'm not sure what to do.

You do not need your pediatrician to make a referral. You can do a parent requested evaluation through Early Interventions at no cost to you. Look online for the contact information for your state. Waiting for the pediatrician and then paying out of pocket for the co-pay or the whole thing if you don't have insurance is not necessary.

Have his pediatrician make a referral to see a speech language pathologist and a hearing screening. Sometimes young children specially boys have a harder time talking. This does not mean they are autistic, it might just be a developmental delay. The sooner he starts getting help the better the outcome will be.

At 20 months you state Early Interventions will evaluate and provide speech therapy for your son. You can get the contact info online or by contacting your local public school department of special education.
The first thing to evaluate is his hearing and then a complete evaluation can be done. My son at 24 months had five words that he'd only say if we coaxed him to say them: dog, mama, hot, no, cat, more. He had speech therapy for 20 months and completely closed the gap.

Just because he does not talk does not mean he is autistic. Actually I would first look at his hearing. The shaking and opening and closing of his fits would also suggest a hearing issue. Have his hearing and speech evaluated....AEA through you local school can do this for free. My guess is that there is a hearing issue and once that is fixed his speech will follow.
