It's a bit hard, until they're big enough to feel the movement. And then them moving around will be what lets you know they're okay. The only time one miscarries, without knowing it, usually, is before they even know they're pregnant. Which is far more common than most people think, and when most miscarriages happen. Think one in every four pregnancies, I believe the figure is) Once far enough along to know you're pregnant a miscarriage is generally quite painful, and there's usually blood involved as well. Basically like a supremely bad period. This happens most of the time because the body realizes something isn't developing correctly, and it's the body's natural way of flushing out an improperly developing fetus. [Usually has to do with the heart.] Or could be the mother has poor health, which puts the pregnancy at high risk.
The only other time most don't realize they miscarry is if they don't pay attention to the baby's general activity level in the womb, and don't realize they're not feeling any movement going on, or a noticeable decrease in movement. And then they go for a check-up and there's no heartbeat. So do pay attention every day to that.
Generally, unless you're at high risk for miscarrying due to health reasons, you will most likely be okay, and thinks will most likely go well. Relax and enjoy the pregnancy.
