She does not test well, and has trouble understanding what the question is asking her to do. She also has a bit of trouble reading.

I was a teacher for eight years and the best way is to help her study 5 to 10 minutes at a time. and for reading. when you read with her get into it and act it out , and this will get her more motivated to wanting to read more .
try having her read a book every night before bed. this will bring her reading level up .

You may also need to have the child evaluated for a learning disability. Could be that she doesn't understand because something is jumbled up in her brain.

You should work with her on reading. Find something she's interested in reading about, even if it's just some silly stories. Reading is fundamental and will slow her progress further if not addressed. Get some easy story problems online and go over with her how to figure out what they're asking. Schools can't always give kids individual attention to overcome difficulties, so the parents need to step in and help their child.
