A child of this age should be seen by a doctor as a high temperature is a sign of an infection. Many children have a recurring temperature with an ear infection. Just to be safe get him seen today.

Use lukewarm water to cool him down - with a face washer or in a bath - as cold water will make him shiver which will raise his temp.
Keep just a nappy & vest/singlet on him - it's important to keep his chest covered so he doesn't shiver.
Put a fan on him to cool him down & offer him plenty of fluids - milk only is best or some water if he has had it before.
If his chest & head feel hot but his hands & feet don't & his temp is over 38 degrees celcius/100.4 degrees fahrenheit then maybe give him the reccommended dose of baby panadol (e.g. if the bottle says 1.2-1.5mls always give the smaller dose of 1.2mls).

If it is 100.5 I would go to the hospital to be on the safe side. At 4 mos. old you can take any chances and you aren't supposed to give them medicine without a doctors permission.
