My 19th month old has a double ear infection and just left the dr. they put him on antibiotics he said that the right ear was really infected. Should i worry about this rupturing im giving him tylenol right now for the pain but how will i know if it ruptures and what should I do.

Can't do anything for a ruptured ear drum. They heal on their own. Just if they do, don't submerge in the bath and don't go swimming. It takes about a month to heal. I wouldn't worry too much about it rupturing though from an infection, this is more common with flying, scuba diving (major pressure changes). I certainly wouldn't fly with him til he's better. The antibiotics will make him start to feel better in a couple days, just remember to give him the whole course. Kids are more prone to ear infections due to their physiological anatomy of their eustacian tubes (where the inner ear drains) being more parallel than vertical. Just keep an eye on him in the future for more infections and treat if you notice any signs. Good luck!
