For starting one or once you're running one?
I was in a teen moms program for a while. I found it was really helpful. You need to have consistant people willing to come to the program, a good location for it to be run. Try to run the program two/three times a week and possibly even a 'teenage dads' program once a week.
A monthly supper group is good. Going on a trip to a certain park/splash pad in the summer is fun. Not so realistic in the winter. Have resources brought in for schooling/job searches/childcare resources/housing etc. Resume building, crafts, parenting skills. More than that have people who care about the teen moms and can help them in their daily lifes. Possibly have an hour or two specifically provided for 'homework night' for those in school/college etc.
I'm not 100% on how you would get the funds to run this type of program but it's a great thing to run. I would also say to allow people who are 13-21 into the program but allow them to stay until they are 24 since it is a really good resource to have. Most of the people who come in would probably be 16-18 but that's just in general.
