My son's dad signed paper work giving me sole custody, and he added in there that I can not contact him for anything except extreme circumstances. My son has his last name, but with a dad that wants nothing to do with him should I let him keep the last name. His father and I were never married. Even in the event that something happens to me he still does not want him. I have complete power to do anything everything for my son, but I am struggling with this decision. Please help, advise needed.

I would do it. It sounds like that is your gut instinct as well, or you wouldn't be asking. Trust your gut.
It's too bad that your son's father isn't interested in him, and if you're the only parent then you deserve to share your name with your child.
Good luck.

i changed my kids names when i split from their dad, especially as he has already started a new relationship and was planning to marry her \9 i refused to have her sound more like their mum than i did). i cannot change the names on the original birth certificate but for everything else including passorts they have my surname which has come in handy since i married as my husband took my name and obviously our new baby shares the name now. its up to you hun, do what you think is best and good luck
