Skateboard/scooter constant ride - marked/damaged toe?

My son is 12. For years now he rides skateboard and scooter. And he does it every day. Recently we noticed strange skin wears on his toe, the one on the foot he uses all the time to push himself when riding his gear! Any similar experience and what to do about it? He uses skateboard shoes which are designed for this type of exercise and constant strain and wear, but on the inside of the shoe I didn't notice any dents or unusual deviations. Could it be caused by something else but friction and repetitive moves? I'm worried about permanent damage and/or fungal infection due to the skin damage. Any moms with skateboard-crazy boys have an idea?

You might want to talk to a doctor and get a medical opinion on whether it will cause more permanent damage to the toe/foot.
