Sleeping schedules for almost 5 month old??

I need help!
I've been having a terrible time trying to figure out how much sleep my son needs. He is almost 19 weeks old and sleeping has been pretty bad lately.
He is EBF and been waking alot more at night.
We try to put him to bed by 7 or 730 everynight.(I nurse him to sleep)
FIrst, he wakes within 45 min of me putting him down so I have to nurse him briefly again.
Then, he usually gives us 2-3 hours before waking up for a feeding, but after that it's all over the place. He's been waking up every hour -2 hrs and once it's 3am- he's very restless and waking up every 45 min-an hour, so by 6ish we're up for the day.
He's still sleeping in our room due to his acid reflux being kind of bad. I thought that was the problem for his sleep, but after putting him on new meds, I don't think that's the case.

His naps are bad too. He'll only sleep if I hold him and he'll give me one good nap in the a.m, then short ones the rest of the day.
I read that babies his age need about 15 hrs of sleep per day, but that you should put them down for a nap after 1.5 -2 hours.
My son is spent after 1.5 hours and looks overtired.
If I have to put him down for naps that often, he'll get way more than 15 hrs of sleep!!!
Any advice would be appreciated, as we are totally exhausted!!!!
