Son will be 21

He's an adult. Let him live his life. He called you to find out how you are... he doesn't need to stop by all the time.
Of course you should get him some sort of recognition for his birthday. He's still your son even if he's an adult.
Not sure about the totaling the car part... did he have to pay for damages?

What I mean by "not sure its possible" meaning I don't know the kind of relationship you have with all of them. Maybe ALL of you (including the parents) should go out to dinner to celebrate his birthday.
If they are judging you for you being a single mom, that is their problem. Try your best not to project this on them. They may not be thinking anything like that. You have nothing to be embarrassed about. This is the choice he made.

He is your son. At the very least, give him a birthday card and a call. Maybe take him and his girlfriend out to dinner all together. That would be nice, but I don't know if it is possible. Not really sure how close you all are. He needs to be very careful with anyone under 18. Depending on the laws in your state, it could be illegal. Other states require consent to be at 16.
