Struggling grade 3 child

I need some advise. My son is 8 years old and is in grade 3. He is struggling in math and when I pick him up in his lesson in the evenings he guesses at times when I ask him basics math. He makes simple errors like when he is counting backwards from say 90 in 2's. He does not check back his work and his writing is big because he thinks he have so much space to print.
He is a good reader, his spelling is well but when he have to write on paper he make spelling errors. I feel like I am such a failure to him....I will go through his social studies with him and when he got back his test he got 9.5 out of 24 and math on patterning a D. He take long to do his test in school , stayed back for a recess to complete his patterning test and place value test.

I get frustrated with him when we go through his math and at times I come down really hard on him. Now I notice when he is writing something he is starting to realize his mistake and corrects it. But it is not enough as he is in grade 3. At the teacher suggestion..constant repetition is needed like kumon. I have found a math teacher where he will go twice a week for I hr each to help him in math.

He's goes to swimming and is a brown belt in taekwondo. I don't think I am nit picking he is not very social able like first approach an unknown person. He responds when spoke too but is a bit shy. Good interacts with friends and families. He plays well but just his academic is low and I don't want him to be left behind.
