No, not necessarily. They may do it again, but not because you were done before. Hopefully they'll just leave you intact and allow you to tear if your body needs more space to allow baby to pass. Tears heal faster than episiotomies.

Get a doula to help you go through it without an episiotomy. Like Sarah Klauzer said, it will heal more easily and the least body trauma the better, especially with an older child. Also, giving birth in the water can make it easier not to tear. Speak with a midwife before you run to an OB, because you can pretty much avoid another episiotomy with the right support and preparation. Good luck!
