What can I do to help my 4 year old to stop having pee accidents?

My 4 year old has been doing really well with potty training and having no accidents for months up until recently. I am not sure what is causing this. He has accidents at school, home, and in his bed. Lately, it has been every day. There have been no significant changes in our lives at the moment. I am currently pregnant, so not sure if that plays into it or not. He has been on antibiotics for nearly a week, but other than that, our routines are the same. Rewarding and taking away things seem to have no effect. I talk with him and he says he will do better and understands, but still has accidents. He will not answer me when I ask him "why or what happened" because he already knows he did something wrong. I can see that in his face.Can anyone please give me some advice on how to handle this and how to get to the root of the problem?

My first thought would be a possible UTI. My second thought is... he's a kid and these things happen. Instead of trying to punish him why not just let him be responsible for changing his clothes and wiping up any mess and just let it be?
