Some states have an actual FORM that can be filled out...I am in Georgia and you don't have a form here...you have to have a nice letter typed with all the information in the letter...like a 'request'.

Below is a webpage that might help you...the path is going to be different for each state.

Just ask your doctor, they should be able to help you out with it.I wanted to add that I agree with Laura, trust you instinct, we mums have them for a reason.

Go to your drs office. I think that's where you get them. Also look up online your state law.

I agree, look into the waiver, medical waivers are legal in every state. Religious ones in most states, and personal reasons in some states.
Don't risk it if it feels wrong. Mom instincts are supposed to protect our kids!

you can get a waiver to sign. my daughter doesn't have mmr vax either. she went to school all year. they just need that paper on file. can ask school or doc office for form.

Actually, if he's had a bad reaction to vaccines in the past you should be able to get the medical waiver. Talk to his doctor about signing the medical waiver for vaccines.

In the UK it does not matter if your child has been vaccinated or not but normally 99% have been. My sons did not have there MMR booster jabs until they were on there way to college and uni because then the MMR booster had just come out and I was not sure about it. The best thing to do is ask your school what there policy is.

in australia you just have to apply to not get them done, cant remember how it all works, but I got letters saying if I werent planning on getting my daughters injections what I needed to do. You dont have to vaccinate to send him to school, you just need to get it all oofcially signed off on.
