when I have a newborn it's roughly one for every hour I'll be gone plus an extra emergency diaper ;)
When they get older, (depending on when you start solids/scheduled feedings, etc) I would bring two or three for an afternoon/evening, unless it was going to be an all day affair.

Unless we were going to be out all day, I usually kept no more than 2 or 3 diapers in my bag, and often just one--I'd use a diaper or two, and forget to put more in the bag when we got home. Also, for grocery shopping and short trips to the park, after age 2 I usually didn't bring a diaper bag--just some water for her and me and maybe a small snack. I'm about to have #2, and I don't really look forward to carrying a diaper bag around all the time again.

I always carry 6 diapers per child. My oldest (3) is in the middle of potty training, but I don't quite trust him when we're out :P so he's in pull ups then and I carry 6 extra. My 1.5yr old is also (obviously :P) in diapers and I always carry 6 for her too.
I just generally have a backpack with me. 1 pack of wipes, 12 diapers. Maybe 2 sippy cups if I don't know if there's going to be anything to drink. And some snacks if I know there won't be food. Don't need much else :)

It's such a relief to have a smaller bag these days! I usually just stick a diaper in whatever bag I'm carrying for my 2 year old. All I need is a diaper, some wipes (and I don't always do that either if I'm not expecting a dirty diaper), and a toy or cracker for emergencies. Such a difference from when I basically needed a packmule to carry 5 diapers, lots of wipes, a change pad, a sling, toys, a change of clothes just in case, formula, safe water, all the bottle stuff, baby spoons, baby snacks, bib, food mill for mashing things, blanket, stroller, etc for a 2 hour outing. Now he walks, he can drink and eat whatever I do, he doesn't expel things continuously out his rear, and he is a pleasant companion. What a difference! :)
