Besides diapers and wipes i also takes a few changes of clothes including something a little warmer, sunscreen and hat ( i live in Australia) a toy or two, themomiter, reciving cloth to feed with, sippy cup of water. I used to carry everything with me where ever I went but now that my daughter is one I usally pack the big diaper bag and have it in the car but take a diaper wipes and cup into shops with me rather then carrying everything. Best thing to do is after a month or so go through it and take things out that you really dont use

I always pack 1 diaper for every hr I plan to be out, you never know if and when baby will have explosive diarrhea. More then enough wipes. The plastic changing mat or the disposable ones.
2 onesies, 2 outfits, socks, hat.
An extra shirt for you and nursing pads if you are breastfeeding and or in case baby spits up.
A few toys, extra binky if baby takes one.
If bottle feeding more then enough bottles filled w/pre-measured baby water, the formula container you can buy w/3 slots is awesome! And it can still be used after baby is over a yr for cheerios or some type of snacks. We have the Safety First Travel Safety Kit thing which includes: thermometer, nail clippers (I just bite his nails off less risk of cutting too short or cutting skin!), nose sucker thing, etc.
Hand sanitizer.
Burp cloths and a couple bibs. Desitin (or butt paste of your choice).
Extra receiving blanket/warm blanket.
Mylicon drops, Tylenol, or Motrin (Motrin only if baby is over 6 months old).
I also use my diaper bag as a purse: coupons, ID/License, credit/debit card, cash, keys, cell phone, etc.

Not sure if I missed anything.

diapers, wipes, toys, baby powder, zinc oxide cream, diaper bags, bottle, tylenol, lanolin cream(if you breastfeed), change of outfit for baby, clean shirt for you. I prepare for the worst!!
