If I don't know when my last period was, will my doctor do an ultrasound at my first appointment?

My birth control pills have messed with my period for the past year and I have no idea when the last "real" period I had was. I bled heavily for a few weeks this summer and I've had light spotting a few times since then. I took a pregnancy test in mid-November that was negative.

This week I've gotten two very clear positive tests and I have a doctor's appointment next week.

Three weeks ago I had light cramping that I thought was my period (but wasn't). My boobs have hurt for about two weeks. And at the beginning of this week I started to feel sort of nauseated. Based on my symptoms, I'd say maybe 6 weeks, but I don't know for sure.

When I was pregnant with my 3 1/2 year old daughter, my periods were very regular and at my first appointment they told me my estimated due date based on that. If they don't have that to go by...will they do an ultrasound, or will they still wait?
