We had sex 3 weeks ago, stupidly unprotected

(it was supposed to be a week before I ovulated) but now my breasts are full, aching, I feel the "let down" and I can express milk. It's been 10 months since I sadly dried up.
We have a living 3 year old who was in the NICU for 8 1/2 months, I pumped (every 2 hours daily) for 8 months then naturally dried up.
After I dried up with her, I didn't even get a drop at all until I was 2-3 weeks with her sister. Her sister sadly didn't survive and I pumped 1-2 times a day for 4-5 days when I got engorged only. Now I've been dried up for 10 months and last week I felt heavy, full and tingling- this time I massaged (like I always do because I get sore near my period a lot but never get milk) and I got colostrum just like I did when I was pregnant with our second daughter. This time, it's been going on for a week off and on (last time it was just a day)
I have been taking B-12 supplements but switched to B-complex with Folic acid and I was taking Iron but quit.
I've also been constipated (bad) for the past 5 days (I have been off the iron for around 6 days)
I've been moody, headaches worse than usual and nauseated after eating most foods. I was recently diagnosed with the MTHFR A1298C homozygous mutation which is why my older daughter was so sick and why her sister died (anencephaly)
I was set up for my first appointment to set up a time to get my tubes tied but chickened out, we have been using condoms because most birth control doesn't work for me. We stupidly thought we were safe just one time but it only took us 2 tries to conceive our second and by the time we had been together for 4 months, I was pregnant with our first so we seem to have high fertility together. If we had sex 3 weeks ago, when would be the earliest time I could test? I've taken several but they mostly said to wait until the first day of the missed period, I'm normally irregular and I'm not sure when it would be (it should be soon, we had sex the week after my period ended so it should be starting soon) I used First response, 3 wal mart tests (said they're accurate on the first day of the missed period) and one from a local pharmacy which was less sensitive. I've now been lactating off and on for a full week and still feel the engorgement pain. I am planning on calling my GYN to have a hormone test done and thyroid (and pituitary gland). What are other tests I should ask about?

I've had multiple negative tests (nearly 10) including 3 first response) but the only time I ever lactated outside of pumping (couldn't bf either girl- 1 was in NICU with tubes and 1 died) so it seems like I am but with all the negative tests it seems I'm not- first response told me around 2-3 weeks.

What are other causes of feeling engorged, feeling the let down and getting milk? I have an appointment next week and I need to know what to suggest.
