if i remove the diaper, my 2 year old often cries scratching his anus. Anybody with an idea what this could be?

could be threadworm - very easy to check as when you look you will see small cotton like things that wriggle. Usually best time to look is at night.

Have you took him to the doctor? If it's red around his bottom it could be a yeast infection boys can get a yeast infection my son had it or it could be worms, pin worms my nephew had this before. But if it's been going on for a while I'd have the doctor to take a look.
