14 year old son's grades dropping..

My 14 year old is a freshman in high school and just finished his first quarter. In middle school he maintained As and Bs and had straight As at one point in 7th grade. This year his grades have dropped quite a bit, he had 2 Cs and 2 Ds. His only 2 As were in PE and Word Processing! He's a good kid and doesn't get into trouble at school or at home. But he is kind of on the lazy side, he does not have a whole lot of ambition to do much of anything, including school work. He's not putting forth any effort with school. He either doesn't do his assignments or he does a half-a•• job and will than get scores like 23%!!! When I ask him about the missing assignments he will say "I'm redoing it for a better score" or "The teacher lost it so now I have to redo it" neither of which are true, on the poorly graded assignments his answer to why he did so poorly is always "I don't know". He has now been grounded from his phone, xBox, and the TV until his grades improve. I do not expect him to get straight As, I expect him to apply himself, do his very best, and strive for those straight As. I'm also realistic and completely understand if he gets a C after he really tried his hardest but struggled with the course. His problem isn't that he's not understandin the work, it's that he doesn't apply him self or do the work. I ask him every evening if he has homework and he'll always say no, but than the next day when I get his daily grades emailed to me there will be missing assignments from the day before!!
I don't know what to do with him or how to help him. Any suggestions would be greatly appreciated.

Hello may I suggest taking him to be evaluated my son had same symptoms I didnt want to but now hes focusing better
