The moment the results come back you have a court date where he signs a paper accepting the results and once that is filed his name should automatically appear on the birth certificate when you go have a new one printed out. At least that is how it worked out for me.

Once the paternity test reveals that he is the father than yes, that would warrant the father's name to be on it.

I don't know whether it will put it on the birth certificate or not, but it will be in the courts record whether or not he is the father.
