I would rethink the child having your exes last name. It makes things very difficult down the road.

He has to be at the birth and sign the birth certificate for his name to be on the birth certificate.Also,why would you want to give your child your ex's last name?That is so stupid,and all you need to get child support is a dna test from him.You can give your child any last name you want,but you will surely regret giving that child his last name later on being that he's not with you and might be with some other woman now.

that not how it is in montreal quebec because i put the father name on the birth cerificate and because the father did not sign it return to me it said if father dont sign put father unclared so that what my child birth cerificate said father unclare it makes me sick to see how singlemoms and unmarried people is treated and being treats in montreal quebec mother left to raise the kids by them self and fathers get a free ticket to walk away and dont look back and kids have to grows up without a father because because the laws is not making father takes respondablies all the stress and presure is on the mother and when the kids growup bad the blame the mothers God please come and help singlemothers no one is on our sides...

Your best bet is to contact a lawyer for questions. However, I believe it's the same in all states in that if you don't put him on the birth certificate, child support is not an option unless you arrange it privately. Also, if there isn't a father on the birth certificate, that will be an issue if you ever need state assistance for any reason. State assistance is considered an "advance" on child support so they would have to have a father on record in order to garnish his wages and keep the child support. But if none of that is a concern, I believe in my unprofessional opinion, that you can name the kid whatever you want and put anyone's name on it for the child's name. You can put any last name you want, to my knowledge there aren't lawful rules as to who you can name your child after.

Start by calling the hospital you will deliver at and getting the birth certifate department they should know the answer! :)

Not sure if it's the same in every state but in Florida the father does not have to be on the birth certificate in order for the child to have his last name....I know several people that gave the child the father's last name and he didn't sign the birth certificate
