Is there a possibility that diet could be an issue? Has his diet changed since his birthday? Any new foods/ additives/ colourings? If you think this may be a possibility to there is a book called Fed Up with Food by Sue Dengate. She also has a very informative website.
Is it possible that it could be anxiety? Maybe related to something that happened around this time? (Some food intolerances may also increase anxiety)
You may find it helpful to have him assessed by an experienced psychologist or even just to chat with one to give you an idea of what the possibilities might be and maybe help work out what is going on and strategies to help.
