I always use Desitin extra strength. My son recently had a rash just like what your going through, He really really did not wan me to come near him to even change his clothes cause he thought I was going to have to change his diaper. I put this on him at night befroe bed and when he woke up he was completely cleared. It is kind of pricy but a little goes a far way and the store brand rash creams dont work.
If you can do it, let him run around without a diaper, ther air will help and after wiping him give it a minute and let him completely dry before putting another diaper, this really helps prevent it.

If it's a diaper rash, I would recommend vaseline or unrefined shea butter. My son had (still has) sensitive skin. It's the only thing that would soothe him and not make the rash worse. Personally I probably would just use the shea butter.

this sounds more like a yeast infection than diaper rash. Ask the pediatrician but in the meantime you can use monistat cream until you can see the doctor.

What I use is Lucas pawpaw ointment. Also how old is your son cos some foods like berries cause rashes and need to be stopped if rash occurs. Also air the area as much as possible and have lots of no nappy time but cover penis area so no wee goes on the rash cos it stings and frequently change nappies making as loose as possible. Good luck.
