Do you usually make your own bread or buy it?
If you make your own then puree your fruits and veggies and incorporate it into the bread dough.
It;s worth a shot and gets her eating at least healthier bread. I'm sure it's fine, ny daughter went through a phase where all she would eat was mac n cheese - she was about the same age.
A "No Thank you" bite is always good too, she has to have at least one bite of everything else before she can say she doesn't like it. I still do this with my daughter (she's 15 now) and hates beans in any form. We have them often since everyone else like them. Each time she has to have at least one bite of them. Hey taste buds change as you grow so you never know when you're going to like something you used to hate.
Good Luck

Well first, I find it funny because I can not get mine to eat bread for ANYTHING.

Second, get her on a vitamin if you havn't already.

Third, start experimenting with food that has bread to get more into her. For example, hummus to dip the bread into, quesadillas with cheese and beans in it, french toast to get some eggs into her.

Require that she tastes other food she has for dinner first before she can have her bread. For me tasting means at least licking it. Take her to the grocery store to help you pick out foods she can try. I have also heard the children who grow gardens are much better at eating their veggies since they are excited about it.

Have you asked her why? Discussed why we eat different foods? She's 4 so even tell her that you worry about her because of it.

Have you talked to her doctor about it? For all we know it could be some health problem causing it?
