I would start with one or the other at first. I personally would do away with the pacifier first. Throw all the pacifiers away except one, then each day cut a piece off of it until there is nothing left to suck. Buy her a cool new toothbrush and tell her if SHE throws her pacifier away she can have the toothbrush. Not only does it get her excited about brushing her teeth but when its time for bed remind her that she exchanged her pacifier for a toothbrush and have her brush her teeth. I'm not sure why but when the kids do the actual throwing away of things it is easier on them.

I just "forgot" his cup one night, told him to go to sleep and I'd get it later. No more cup.
