In need of some advise with my 5 year old and his pooping issues

So, my son is five years old and since he was almost 1 he has had severe constipation issues. He was sent to a GI specialist at 18 months-ish, and she put him on Miralax which helps a lot but he still always has a harder time going and is definitely not regular. He started Kindergarten this year and has started pooping his pants. He has 3 times in school so far and once at home. I am at a loss on how to handle this and what to do. He was an extremely easy potty trainer and was potty trained by the time he was 2. He says that he is nervous about it hurting at school which i understand bc it always has hurt him to go. I even set it up with his teacher and his nurse to have him go to the nurses office if he has to go, but he just doesn't say anything till it's too late. And even then he won't say anything. I am considering talking to his pediatrician to see if maybe she thinks maybe he needs to be seen by the specialist or if she maybe has any ideas. But i just do not know what to do. Have any other moms had this issue with their kids?If so How did you get it to stop?
