No, it means your pelvis is narrow. Plenty of narrow pelvises have pushed a baby right on through without a problem. You cervix is not dilating yet, which since this is the first baby is not uncommon. As you reach 40 weeks, if your are not going into labor on your own your doc may suggest induction. You don't have to, but it would be an option. If your doctor believes your baby will not fit, he may say you need a c-section. Again, this is your choice. You could tell him you want to labor and see if you are able to deliver vaginally. If after hours of labor you are not dilating, then it would be time to consider a surgical delivery.
I think doctors make comments when they do an exam sort of without thinking about how the patient may interpret their statement. I remember once my doc commented that my cervix was tilted a bit, I obsessed about that meaning I could not conceive. Why would he say it if it wasn't significant right?
Write down your questions so when you go next week you can ask your doctor to explain his opinion more clearly for you. Good luck!
