Trying to get 18-month old to sleep in her own bed in her room...HELP!

My daughters pediatrician recommended we get our daughter in her crib asap, but every time I try to put her in the crib she cry's so hard she vomits and chokes. I explained this to the doctor, and she said that was OK! She said to put her in the crib and let her go for 5 mins, check on her, dont go back for 10 mins and so on. We tried to do what she said but our baby actually crawled out of her crib and was standing at the door knocking with puke on her. We were all three devastated. WHAT DO WE DO?
